Report a Side Effect

If you believe that you or someone in your care has experienced a side effect associated with a Dormer Laboratories product, please let us know. Your report helps us and health authorities monitor product safety and protect others. We accept reports from patients, caregivers, and healthcare professionals.

If the reaction is severe or life-threatening, seek immediate medical attention and contact local emergency services before reporting the reaction to us.

What We Need From You

To help us assess and process your report as quickly and effectively as possible, please provide as much relevant information as you can. At a minimum, please include:

  • Product name (brand name and/or active ingredient)
  • Lot/batch number and expiry date (if available)
  • Initials of the person who experienced the suspected side effect or adverse event, and other identifiers such as age, sex, or date of birth
  • Date and time the event started (if known)
  • Date(s) of product use and details regarding how the product was used (if known)
  • Description of the suspected side effect or adverse event, including symptoms, severity, and outcome
  • Any other medicines, natural health products, or treatments being used at the time of the event (if available)
  • Your contact information so that we may follow up if additional information is required
